**This form should only be filled out by the Business Unit Compliance / Anti-Corruption Officer**

This form requests basic due diligence information about the Intermediary you are proposing. All fields are required to be completed. Please contact [compliance@marmon.com](mailto:compliance@marmon.com) if you have any questions about completing this form.  
As soon as you submit this form, an automated email will be sent to the Intermediary that introduces Blue Umbrella and the anti-corruption due diligence process as soon. A copy of that email can be found in the Blue Umbrella training guide.
